How it Works

STEP ONE: ACCESS

James visits a community organization or submits a request form online

James completes a comprehensive assessment and identifies Substance Use Disorder needs

STEP THREE: REFERRAL

A Peer Professional uses the Kyros platform to gain consent and refer James to service providers and resources that meet his unique
needs

STEP FOUR: CARE RECEIVED

James receives the care he needs while interactions and outcomes are tracked in the Kyros Platform

STEP FIVE: FEEDBACK LOOP & OUTCOMES

James and his care team receive real- time updates on recovery progress. Opportunities for James to become a peer provider after 1 year in recovery
